How to remove codeigniter index.php

The information provided in the tutorial in codeigniter 2 seems to give several people problems when trying to remove the index.php from the urls.

RewriteCond %{REQUEST_FILENAME} !-f
RewriteCond %{REQUEST_FILENAME} !-d
RewriteRule .* index.php/$0 [PT,L]
